New Form: CPS Cycle 2 Exempt Modificaiton Form

We’ve created a new Form to support multi-cycle action research projects in CPS programs. The Cycle 2 Exempt Modification Form is designed to assist PIs and students streamline the decision to decide if cycle 2 activities being added to a study requires submission as a modification and additional review from the IRB. This Form is meant only for those who have received an exempt determination letter for cycle 1 of their CPS action research.

The new form does not change any expectations related to review but is rather designed to set researchers up for success by assisting them in determining if their activities do require IRB review and approval approval of additional study activities or if they can be implemented without the need to submit a modification to the IRB.  This would significantly decrease administrative burden and speed up the expected time to implement Cycle 2 activities of action research projects if students and PIs are able to determine that a modification does not need to be submitted and reviewed.

This completed form should be kept in your protocol file or study records if study teams determine that Cycle 2 activities do not require a modification to be submitted. This Cycle 2 Exempt Modification Form is now available on our Forms page.

Additionally, though this is intended as a resource for researchers, we are happy to meet with PIs who have specific questions on how to use the form. Please feel free to join one of our drop-in hours for guidance.
